Electronic Communications Authorization

  • WRITTEN CONSENT FOR OPT-IN BY MEMBERS TO USE OF ELECTRONIC NOTICE & ELECTRONIC VOTING. THIS FORM IS EVIDENCE OF THE MEMBER’S INTENT TO OPT-IN TO RECEIVE AND USE ELECTRONIC NOTICE AND ELECTRONIC VOTING THIS IS A WRITTEN CONSENT FORM WHICH SHALL REMAIN VALID UNTIL DULY REVOKED BY THE MEMBER. The undersigned, being one(1) eligible voter for the following property:

  • at Remington Master Homeowners Association, Inc("association"), hereby consents to receive electronically notice and communication for the Association to the fullest extent permitted by law (pursuant to Section 720.317 of the Florida Statutes, as amended from time to time), and to receive notice of Association business via electronic transmission (pursuant to Section 720.303 of the Florida Statutes, as amended from time to time). I, the undersigned owner, affirmatively act to OPT-IN and receive any and all future Association notices via electronic means at the email address provided herein. I further acknowledge that by opting in that I have the ability to vote on any matter before the Membership via an electronic vote and that I am responsible for obtaining electronic access for purposes of receiving notices and for casting electronic votes. Should I wish to revoke my decision to OPT-IN as set forth herein, I understand that I must do so in writing and as otherwise proscribed by the Electronic Notice and Voting Resolution. I have reviewed the matter and hereby: OPT IN and request electronic notices to be sent to the following valid email address [please provide only one email address on the line below]:

  • To the best of my knowledge I consent that all information is true and accurate. Having completed all required information, under penalty of perjury all above information is true and correct.
  • LEGAL NOTICE: Owner’s Opt-In shall remain valid until the owner opts out of online voting according to the procedures established by this Board Resolution. Specifically, the Owner must notify the Association by and through its property manager in writing executed by all owners of record of the intent to opt out and shall provide the Association and effective date opt out date in that notice of not less than 90 days’ notice. During the 90-day notice period, the owner may still receive notice by electronic means and may vote electronically. Additional Legal Disclosure: Owners who opt-in for electronic notification and opt-in to participate in electronic voting should note that their E-mail addresses used for official communication from the Association become available for inspection by any Member making an official records inspection request.
